There is no extra charge for this service, the order number is the same for all allotted questionnaires, does not expire and you will not be billed for the report until it is processed.

 

When using Windows, the minimum of 98 and IE 5.5 or higher is required. Also, please allow pop-ups for this site.

 

Directions:

1. go to www.personalysis.com.

2. at the top right corner, click on “the questionnaire”.

3. select the first option (start a questionnaire..).

4. at the bottom of the next screen, click on “I agree”.

5. enter your order number N_______ (zero’s not o’s).

6. proceed through the Online Questionnaire screens.  If you are an applicant for the position please check the applicant box.

7. write down your questionnaire I.D. number in case you have to stop and wish to finish at a later date.
